b) Apply and cover fee for Entrance Assessment and submit copy of Report Cards English Language and Spanish.
c) Approve Entrance Assessment NEXT DATE: Tuesday May 13, 2008. 8:30 to 12:00 at Campus Francita
d) Submit required documentation
Documents that will be required by the Jr. High School’s Office during the first week of school:
a) Birth Certificate (1 original, 2 copies)
b) CURP -Mexican citizens only- (1 original, 2 letter-size copies)
c) Up-to-date Vaccination/Immunization Record (1 copy)
d) 6 Pictures (3X2.5 cm, B/W, white background, matte paper, no hair covering the forehead, and wearing the school’s uniform shirt)
e) Last Report Cad (signed and stamped by proceeding school)
f) Elementary School Certificate (1 original, 2 copies)
g) Letter of Recommendation from the proceeding school (1 original)
h) Not mexican applicants: FM-3 Format -Permit to Reside in Mexico (1 copy)
